St. Luke Lutheran Community – Indoor/Outdoor Visitation

Indoor and Outdoor Visitation situations will comply with all visitation requirements of the CMS order.

What You Need To Know:

  • Screening of all who enter the facility for signs and symptoms of COVID-19 will take place and denial can be made of entry of those with signs or symptoms or those who have had close contact with someone with COVID-19 infection in the prior 14 days (regardless of the visitor’s vaccination status).
  • Visitors must adhere to core principles of COVID-19 infection prevention while in the facility:
    • Visitors who have a positive viral test for COVID-19, symptoms of COVID-19, or currently meet criteria for quarantine should not enter the facility.
    • Visitors must perform hand hygiene prior to, during as needed, and after each visit with the preferred use of alcohol-based hand rub.
    • Visitors must wear a face covering or mask (covering mouth and nose) at all times during visit.
    • Visits shall be conducted using physical distancing, however if during visit personal contact is permitted, it must be done so for a limited amount for time.
    • COVID-19 Testing is available upon request.
  • Visitors who are unwilling to adhere to the COVID-19 infection prevention practices will not be permitted to visit or will be asked to leave.
  • Entering into a congregate care setting increases your risk of exposure to COVID-19.
  • The facility must follow CMS, state, and local health department’s orders regarding visitation.

Guidelines:

Once you arrive to the main entrance with your face mask, proceed to the front desk for screening.

1. Each visitor will be required to perform hand hygiene prior to, during as needed, and after each visit

2. Your temperature will be taken and you will be asked to complete a screening form

    Visitors will be screened for fever and respiratory illness using a designated form prior to entry. Those with symptoms of a respiratory illness will not be permitted to visit.

3. Sign in is required in the designated visitor log book.

4. A copy of your federal ID must be provided to facility if you are a first-time visitor.

5. Visitors should go directly to the resident’s room or designated visitation area. Please do not walk around different halls or areas of the facility.
